サルコイドーシスの臨床研究

サルコイドーシスは、全身性肉芽腫性疾患であり、その診断には、組織学的に非乾酪性類上皮細胞肉芽腫の像がみられることが必要である。

皮膚科が係るのは主に、(1) すでに肺や心にサルコイドーシスを疑う病変があり、診断確定のために、皮膚症状の有無についてコンサルトされる場合と、(2) 皮膚病変からサルコイドーシスと診断され、こちらから他臓器病変を検索する場合、とであろう。

サルコイドーシスの皮膚病変は多彩であり、しかも人種差が認められる。また、皮膚病変は生検しやすいため、診断確定のための有力な手掛かりとなる。皮膚科医は、どこにどのような皮疹を予測しながら患者さんをみなければいけないか熟知しておく必要がある。さらに、どのようなタイプの皮疹が、他臓器病変と関連が高いかについても、知っておくことも重要である。

当教室では、これまで数多くの皮膚サルコイド病変を経験し、本邦人のサルコイドーシスの皮疹について報告してきた。特に近年は、教室症例の集計も報告している。
