乾癬の研究

近年、乾癬は皮膚だけの疾患ではなく全身性の慢性炎症性疾患であることが強調され、眼・関節・腸管を始め、心血管系イベントを引き起こす注意も喚起されている。また、その病態の理解も、Th1からTh1/Th17タイプのサイトカインが主役であることや、自然免疫系の活性化、さらに脂肪細胞からのアディポサイトカインも多彩な作用機序か徐々に明らかにされつつある。

治療法も、本邦においては2010年から生物学的製剤が導入され、難治性の病変に対して劇的な効果をもたらした。一方、標的分子の重要性からみた、乾癬の病態解析も飛躍的に進展し、さらに、Adverse effectからさえも、乾癬の病態への重要な手掛かりになると考えられており、まさにClinic to benchという言葉が当てはまる領域で、皮膚科以外の研究者からも注目されている、ホットな分野である。

これまで我々は、HCVが乾癬発症における役割の一旦を担っている可能性や(Yamamoto T, et al. Acta Derma Venereol 1995)、顔面の乾癬病変にタクロリムス軟膏が奏効すること(Yamamoto T, et al. Acta Derma Venereol 2000)を世界に先駆けて報告してきた。また、特殊なタイプ（関節症性乾癬、膿疱性）の症例を報告するとともに、皮膚科からみた関節症性乾癬について症例を蓄積してきた。特に最近では、関節症性乾癬患者の日常生活動作評価と治療との相関（日本皮膚科学会雑誌 2008）や、教室におけるこれまでの40例を集計し報告している（第30回日本乾癬学会で報告）。今後は、関節症性乾癬を早期に発見・診断し、早期に生物学的製剤を用いた治療介入ができるよう努めるとともに、その病態においても様々な角度から解析していきたい。
